Inside the container the default HOME is /root, which uid 1003 cannot write, so codex cannot create ~/.codex and the app-server never answers initialize (host.py times out after 60s, task fails with queue.Empty). Setting codex_home to /app/work/codex-home (mounted writable, owned by codexsvc uid 1003, same as the container user) lets the app-server start and run turns. No secrets in this file.
